Appointments
Appointments are scheduled from 9.00AM to 4:00PM, Mondays
through Fridays. When calling for an appointment, please designate
the reason for the visit, such as an annual exam or an obstetrical
visit. If you need to schedule a particular procedure, please
notify the appointment staff person so that he/she can make the
appropriate arrangements. We make every effort to minimize waiting
time, but the nature of our practice and unforeseen emergencies
will, on occasion, disrupt the schedule. If you are unable to
keep a scheduled appointment, please notify us at least 24 hours
in advance.

New Patients
New patients are asked to arrive fifteen minutes prior
to their scheduled appointment to fill out a medical history and
a demographics information form. Please bring the following with
you: (1) your health insurance card; (2)
referral, if required by your insurance company and; (3)
any previous records that may be pertinent to your continuing
care. You can download and pre-fill the Registration Form and

Form to speed up your processing.

Routine
Visits
Annual and other visits also require some paperwork and
insurance verification prior to the visit. Arriving five to ten
minutes prior to the scheduled visit will allow time for this
to be completed before you see the doctor. Please remember to
have the following with you at the time of your visit: (1)
your insurance information and card; (2) referral,
if required by your insurance company.

Emergencies
If you experience a problem that requires immediate attention,
please call our office and request an emergency appointment. Our
staff will do its best to have you seen on that day.
If you are not certain of the urgency of a particular problem,
our nurse will be able to evaluate your situation and make recommendations
for an appointment.
If you find yourself with an emergency on the weekend or when
the office is closed, please call the office phone number and
you will be given instructions for calling the answering service.
The physician on duty that evening will be notified by the service
and return your call. If for some reason your call is not returned
promptly or you feel that your problem is life threatening, you
may proceed to the hospital emergency room directly and the staff
there will notify the physician when you arrive.

Answering
Service
If you have an emergency when the office is closed, our
answering service will assist after hours and on the weekends.
If you are experiencing a medical emergency or you are an obstetrical
patient in labor, please contact the service at the number provided
when you call our regular office number. One of the physicians
is on duty 24 hours per day to handle emergencies and take care
of obstetrical patients in labor. Because the physician on duty
may be in surgery or delivery, your call may not be returned immediately.
If you place a call to the answering service, we suggest you remain
at home until the call is returned or provide a number where you
can be reachable.
If you are experiencing a medical emergency and do not hear from
the doctor within 15 minutes, please notify the answering service
and proceed to the hospital. If your call requires a prescription
to be called to the pharmacy, please have the phone number of
the available pharmacy. Routine prescription refills and questions
regarding treatment and medications are handled during routine
office hours, and are not handled by the physician on emergency
duty.

Lab Reports
PAP smear results are available two weeks after the test
is taken. You will be notified only if the test is abnormal and
needs follow-up. For other test results, please leave your name,
phone number, and test name on our lab result line. We will call
you back with the results.

Prescription Refills
We ask for your help as we try to improve the efficiency
of our office for prescription refills. Please review our policy
if you need a refill on a prescription.
- At the time of your annual examination, you will be given
a prescription for all maintenance medications (hormones, oral
contraceptives, vitamins, etc.). The prescription will be written
for a 90-day supply with three refills. Please be sure you have
all the prescriptions you need for the year before you leave
our office.
- Please note prescriptions cannot be refilled if it has been
more than one year since your last visit.
- If you change pharmacies or insurance companies, it is the
responsibility of the pharmacist to transfer that prescription.
If the pharmacist questions the availability of refills, please
inform him that your doctor has written the prescription for
one year.
- The physician on call at night or on the weekends will not
call in routine medication refills. Please plan ahead of if
you are traveling or going on vacation. Controlled drugs can
never be called in after office hours or on weekends.
- Prescriptions for emergency medications will be called in
to your pharmacy by the physician on call. Please have the pharmacy
phone number available.
- Long distance calls to pharmacies may incur a service charge.
- Please be aware that your pharmacy may restock medication
that is not picked up within several days of your refill request.
- We are not able to refill a prescription that is written by
a physician from another practice.
